Age Requirements / What to Bring
- Must be at least 16 with legal guardian consent
* Minor must have birth certificate, state issued ID, driver license / permit, or passport.
* Parent must have state issued ID or passport, if you are a legal guardian you must also provide court documents.
Same Day Appointments (Walk ins)
- Same day appointments are welcome, we recommend calling in advance for openings / availability
- Same day appointment openings are not guaranteed unless scheduled
- A deposit must be made when scheduling a appointment

Cancellation / Rescheduling Policy
- All appointments require a deposit to book
- DEPOSITS ARE NON REFUNDABLE
- You can reschedule an appointment ONCE before losing the deposit, must be a 24 hour notice
- A 24 hour notice must be given in order to reschedule an appointment, the deposit is surrendered otherwise
