- State issued ID / drivers license
- Military ID / military dependents ID
- US Passport
- Passport card
- Valid national ID card
- U.S. permanent resident or other identification card issued by U.S. Citizenship
- Birth certificate along with a picture ID
What to bring for minors
- MINOR MUST HAVE ONE OF THE FORMS OF ID LISTED ABOVE.
GUARDIAN MUST BE PRESENT WITH THEIR FORM OF ID AS WELL -
5.45.040 Age limitation/consent.
(A) Tattooing or body-piercing may not be performed on individuals under 18 years of age without parental consent.
(B) A parent, guardian, or custodian of an individual under age 18 who desires to give consent to a business to perform on the individual a tattooing procedure or body-piercing procedure shall do both of the following:
(1) Appear in person at the business at the time the procedure is performed and remain through the procedure.
(2) Sign a document provided by the business that explains the manner in which the procedure will be performed and the methods for proper care of the affected body area following performance of the procedure.
